Effects of multi-component exercise nursing intervention on frailty in elderly patients with chronic heart failure

Objective To investigate the effects of a multi-component exercise nursing intervention on frailty,activities of daily living and quality of life in elderly patients with chronic heart failure. Methods Through literature analysis and expert meeting,the multi-component exercise nursing intervention program was formed. A total of 71 elderly patients with chronic heart failure who admitted from January 2022 to September 2022 in a tertiary A hospital in Shandong Province were randomly divided into an intervention group(35 cases) and a control group(36 cases). The control group received usual care,while the intervention group received an additional multi-component exercise nursing training. 2 groups were evaluated by Tilburg frailty indicator scale,modified Barthel index scale and 12-item short form health survey scale before the intervention,4 weeks and 12 weeks after the intervention. Results A total of 62 patients(31 in the experimental group and 31 in the control group) completed the study. Generalized estimation equation results showed that the inter-group effect,time effect and interaction effect of frailty,activities of daily living and quality of life in the 2 groups were statistically significant(P<0.05). At 4 weeks and 12 weeks after the intervention,the Tilburg Frailty Indicator scale scores in the intervention group were lower than that in the control group(P<0.05),and the scores of the modified Barthel index scale and 12-item short form health survey scale were significantly higher than those in the control group(P<0.05). Conclusion The multi-component exercise nursing intervention can effectively improve frailty,activities of daily living and quality of life in elderly patients with chronic heart failure.

Exercise rehabilitation is an important mean to promote early rehabilitation of patients with acute decompensated heart failure(ADHF) during hospitalization. This paper reviews the related research on exercise rehabilitation of patients with ADHF,summarizes the exercise styles,exercise methods,influencing factors and intervention effects,and enriches the intervention content for early rehabilitation of patients with ADHF.

Path analysis of safety behavior status and related influencing factors of newly recruited nurses

Objective To understand the status quo and influencing factors of safe behavior of new nurses,in order to provide a theoretical basis for managers to do a good job in the management and training of new nurses,improve their level of safe behavior,and ensure medical safety. Methods In September 2020,the convenience sampling method was used to select nurses enrolled in 4 tertiary A hospitals in Shandong Province from September 2018 to September 2019 as the research subjects. A general data questionnaire,Nurse Safety Behavior Questionnaire,Patient Safety Culture Assessment Scale,Perceived Organizational Support Questionnaire,Patient Safety Competency Self-rating Scale of Nurses were used to investigate. Results The safety behavior score of 454 new nurses was(56.56±5.19);the safety behavior of new nurses was positively correlated with patients’ safety culture perception(except pressure dimension),organizational support,total score of patients’ safety competence and all dimensions,and negatively correlated with the pressure dimension of patients’ safety culture perception;AMOS analysis showed that patients’ safety culture perception and patient safety competence had a direct positive impact on the safety behavior of new nurses(β=0.246,β=0.048,both P<0.001). Bootstrap analysis showed that organizational support and patient safety competence had significant mediating effects on patients’ safety culture perception and nurses’ safety behavior(95%CI 0.031-0.091). The total effect of new nurses’ patients safety culture perception on nurses’ safety behavior was 0.242(P<0.001,95%CI:0.207-0.277),and the direct effect was 0.094(P<0.001,95%CI:0.049-0.140). The indirect effect was 0.148(P<0.001,95%CI:0.085-0.215),and the mediating effect accounted for 61.16% of the total effect value. Conclusion Organizational support and patient safety competency play a mediating role between patient safety culture perception and safety behavior of new nurses. The hospital should strengthen cultural awareness training for new nurses security,strengthen occupational stress management. The safety cognitive level and safety competency of the new nurses can be improved by the construction of a positive safety culture atmosphere,so as to raise the safety behavior level of new nurses.

This review will introduce the concept and implementation steps of intervention mapping and its application in pre-hospital prevention,in-hospital care and post-hospital rehabilitation. Pre-hospital prevention includes prevention and control of infectious diseases and early screening of diseases,promotion of maternal and child health,control of risk factors and improvement of unhealthy lifestyles. In-hospital care includes promoting clinical application of new nursing technologies and talent training,doctor-patient communication and information exchange,health education and risk assessment. Post-hospital rehabilitation includes self-management of chronic diseases and health behavior changes,postoperative rehabilitation care of patients and cancer survivors. In addition,to promote and better use in China and improve the quality of clinical nursing intervention,we summarize the advantages and limitations of the intervention mapping.

Short-term and long-term clinical outcomes of same day discharge after percutaneous coronary intervention in patients:a systematic review and meta-analysis

Objective To systematically assess short-termed and long-termed clinical outcomes of same day discharge after percutaneous coronary intervention(PCI). Methods The Cochrane Library, Wiley online library, PubMed and EMBASE et al databases were searched for randomized control trials comparing same day discharge and overnight stay in the hospital after PCI. RevMan 5. 2 was used as the statistical software. Results 10 randomized control trials with a total number of 3 908 patients (1 844 patients were discharged on the same day and 2064 patients stayed overnight in the hospital) were included. The results showed that, compared with overnight hospitalization, the discharge on the same day after PCI had no significant effect on 24-hour incidence of adverse cardiac events [RR=0. 28, 95%CI(0. 05, 1. 64), Z=1. 42, P=0. 16] , 24-hour overall incidence of adverse cardiac events [RR=0. 67, 95%CI(0. 17, 2. 68), Z=0. 57, P=0. 57] , 30-day incidence of adverse cardiac events [RR=0. 83, 95%CI(0. 43, 1. 58), Z=0. 57, P=0. 57] , 30-day overall incidence of adverse events [RR=0. 94, 95%CI(0. 71, 1. 26), Z=0. 40, P=0. 69] , 30-day readmission rate [RR=1. 12, 95%CI(0. 76, 1. 66), Z=0. 57, P=0. 57] , one-year incidence of adverse cardiac events [RR=1. 03, 95%CI(0. 76, 1. 40), Z=0. 19, P=0. 85] and one-year overall incidence of adverse events [RR=0. 98, 95%CI(0. 76, 1. 25), Z=0. 19, P=0. 85] . The medical expenses were reduced and the patients’ satisfaction was improved, but the patients were not well prepared for discharge. Conclusions Discharge on the same day after PCI does not increase the occurrence of short-term and long-term adverse events. In the future, it can be carried out in the outpatient services for specific patient groups.

Expert consensus on the discharge planning for hospitalized elderly patients(version 2019)

Objective To establish an expert consensus on discharge planning for hospitalized elderly patients. Methods Studies were retrieved,evaluated and collected by the evidence-based nursing method. Revelent recommendations and research results were selected to form the expert consensus draft. Experts were invited to complete 2 rounds of Delphi expert consultation and 2 rounds of expert meeting. Each item was revised and adjusted considering objective evidence and expert opinions. Results The positive coefficients of experts in 2 rounds of Delphi expert consultation were all 100%,and the degrees of expert authority were all 0.83. The average importance value of each item was higher than 3.5;the variation coefficent of each item was less than 0.25;the Kendall’s W were 0.234 and 0.253 respectively(all P<0.001). At last,experts agreed on 5 dimensions of discharge planning for elderly patients. Conclusion The consensus can not only provide guidance for the clinical practice,but also promote the development of the discharge planning for hospitalized elderly patients in China.

Rumination status and influencing factors in young and middle-aged cancer patients during hospitalization

Objective To investigate the level and influencing factors of rumination in young and middle-aged cancer patients,to provide a basis for clinical medical staff to implement personalized intervention.Methods Totally 373 cancer patients in the department of oncology at a tertiary hospital in Shandong Province were investigated using general information questionnaire,Chinese Version of Event Related Rumination Inventory,and Emotion Regulation Scale.Results The score of rumination of cancer patients was(28.64±12.13),intrusive rumination score was(13.92±6.42),the purpose of rumination score was(14.72±6.80). Expression suppression and religion could explain 14.2% of the total variance of invasive rumination. Expression suppression,family relationship and education level could explain 22.1% of the total variance of purposive rumination.Conclusion The rumination level was low in cancer patients. Expression suppression,religion,family relationship and educational level are important influencing factors of rumination in cancer patients. To promote the development of positive cognitive processing and the recovery of physical and mental,personalized intervention should be taken by clinical medical staff in cancer patients with different characteristics.

Application of one-insertion subcutaneous tunneling technique for Power PICC placement via ultrasound

Objective To explore the effect of one-insertion subcutaneous tunneling technique for Power peripherally inserted central catheter(PICC) placement via ultrasound.Methods A total of 963 patients with malignant tumors treated in our hospital from January 2016 to December 2018 were enrolled,including 493 patients in the experimental group and 470 patients in the control group. The experimental group adopted the one-insertion subcutaneous tunneling technique to place Power PICC,and the control group adopted the routine method. Chi-square test was used to analyze the incidence of complications in two groups;Cox regression were used to analyze the influencing factors of catheter extrusion and obstruction;survival curve was used to reflect the occurrence of catheter extrusion and obstruction in both groups.Results The incidence rates of catheter extrusion and obstruction in the experimental group were significantly lower than those in the control group[aHR=0.304,95%CI(0.137,0.677),P<0.01;aHR=0.304,95%CI(0.097,0.958),P<0.05]. Demographics and disease characteristics had no significant impacts on catheter extrusion and obstruction(P>0.05).Conclusion One-insertion subcutaneous tunneling technique can effectively reduce complications after placement,which is a safe and effective technique to place Power PICC.
